Public utilities are mandated under the CBYD regulations to report
to the central clearinghouse regulatory violations and/or damage
to their underground utility plant. This information is forwarded
to the Department of Public Utility Control (DPUC) for review. Fines
of up to $10,000 may be levied for any violation of any portion
of the law. As such, this Excavator Damage Report should be used
if your markout request was not accurately or timely marked. A copy
of this report should be filed with Call Before You Dig, Inc., 2040
Whitney Avenue, Hamden, CT 06517. The clearinghouse will provide
a copy of your report to the DPUC for review.
